A "Name Change" request is used for those customers who have changed their names as in the case of a divorce, marriage or death. The account billing information remains the same in that no one actually moves from the residence. This request will be verified by phone before processing.

A "Final Bill on Account" is a request to terminate services at a specific residence. All pertinent information must be provided. These areas are marked with an asterisk (*). No request will be processed until verified by the office staff. There must be advance notice of at least one business day when finalizing an account to allow for the reading to be put into the schedule.
